Job Description PURPOSE OF POSITION NYC Health + Hospitals Pediatric Nurse Practitioners (NPs) provide Primary Care services to patients from infancy through age 21 years. This includes diagnosing and treating common Pediatric conditions, educating patients in developing skills and knowledge to control chronic illness, promoting preventive health through lifestyle modification and recommending preventive services. PNPs order and review diagnostic tests and ensure access to timely and appropriate levels of care (e.g. same day appointments). PNPs coordinate patients' care across various locations of service (e.g. emergency, primary, specialty, dental, mental health, substance use, inpatient and at home care). The focus of this role is supporting Primary Care Providers (PCPs) by providing timely access for empaneled patients of the PCPs (e.g. sick visits and same-day appointments).
