Home Infusion Pharmacist Positions Open - Full Time

Landmark Infusion is currently accepting resumes for Production Pharmacists, Clinical Pharmacists, and Pharmacy Supervisors to join our Home Infusion Pharmacy Team.  We have a dynamic team-based environment, provide full time benefits, and seek to make a difference in North Texas. Landmark Healthcare was founded in 1999 and our Infusion Pharmacy was established in 2011. We serve patients throughout North Texas and aim to bring a "whatever it takes" mentality to every interaction with each patient.

The Home Infusion Pharmacist performs or directs others performing the following Infusion order processing and dispensing functions: order approval, patient intake, medication procurement, medication control, medication preparation, quality control, compounding sterile preparations, patient counseling, clinical services, dispensing, packing, and delivery. Staff Pharmacists adhere to all regulations, laws and Joint Commission standards. The positions will assist with and perform a variety of duties as required by the Pharmacist in Charge, including:

  • Reviews and provides final interpretations of prescriptions issued by Physician, or other authorized prescriber to assure accuracy and determine formulas and ingredients needed.
  • Review patient medication profiles and determine homecare suitability for each patient.
  • Develop, recommend, and monitor pharmacy plans of care for patients receiving home therapy.
  • Maintains high knowledge level and proficiency of all policies and procedures pertaining to Landmark Healthcare and Pharmacy Operations.
  • Communicates with pharmacists, pharmacy technicians, referrals, patients, caregivers, and any other entity or person in pursuit of providing safe and high-quality Pharmacy products.
  • Record all conversations with patients in the appropriate information systems.
  • Educate others on the proper production and use of Pharmacy products.
  • Educate patients about proper use, drug interactions, side effects, dosage, and storage.
  • Heavy use of Landmark computer information systems in order to answer questions and manage pharmacy operations.
  • Follow Landmark quality control procedures regarding inventory and supplies, including maintaining required levels of inventory to effectively serve our patient base, determining adequate inventory levels, approving purchases, and requiring the implementation of effective safety and other quality control procedures.
  • Manage and direct others in Pharmacy to maintain pharmacy dispensing operations, including mixing, packaging, and labeling pharmaceuticals.
  • Manage and direct others in Pharmacy to maintain cleanliness, safety, infection control and adhere to standards and procedures.
  • Manage and direct others to complete accurate written and digital records in a timely manner that documents pharmacy activity and other information.
  • Manage and direct others to follow effective infection control procedures, policies and protocols.
  • Provides indirect oversight of pharmacy technicians and pharmacy support staff.
  • Adheres to Universal Precautions as necessary in a home or hospital setting.
  • Ensure that all orders dispensed from Landmark Pharmacy adhere to universally recognized or required standards of practice, quality control, and safety, including, but not limited to: calculations, identifying medication conflicts, assembling and mixing components, packaging, and labeling.
  • Provides leadership to others with positive motivation, goal setting, and training.
  • Observe issues and trends in performance or operations and provide timely feedback to PIC and other Managers for process improvement and growth.
  • Maintain clinical skills and knowledge basis to effectively perform the tasks above. Participates in scheduled training and in-service classes, as required.
  • Performs other duties/activities as directed within the scope of the individual’s training, experience, and level of competence.

Job Requirements:

- Must be a licensed Pharmacist (active and unrestricted license)

- Experience in Infusion settings but preferred, not required

- IV certification is required

- Excellent interpersonal skills

- On-Call is required

- Must be eligible to work in the United States

- Must be able to handle a fast-paced work environment

- Strong communication skills are required

- Must be able to work well with a team of Clinical Pharmacists, Pharmacy Technicians, and other Pharmacy staff

- Bilingual is preferred, not required
